 Please note that most of the forms on this page can be filled-out
online and then printed. They should then be mailed or faxed to our
local office. All forms are provided in pdf format. If you do not have

 Health & Safety
Safety Director - Norm Prevost is our Local Safety Director.
Call on Norm for advise and/or action on WorkSafeBC compensation issues
and plant safety issues.
Safety Committees - Each operation has an elected Safety
Committee. Their duty is to make your operation a safer place to work,
and to help enforce the Worker's Compensation Act of British Columbia.
All unsafe conditions should be reported to your foreman. If not
rectified, contact your Safety Committee member and the Local Union
office. If you are injured, contact your foreman if possible and then
go to the First Aid room. Reporting to the First Aid Room IS A MUST!!!
